Steps in the Evaluation Process
The procedures and information below are similar for evaluations of children, adolescents, or adults.

Application Process
Schedule a Consultation Call
Contact our Intake Coordinator at ekondelis@hanovercbt.com to schedule a time to discuss your concerns with Dr. Van Santen.
Submit Application, Developmental & Academic History
Before we schedule an appointment, it is necessary for us to have background information about the nature of your concerns, as well as developmental and academic history to help us plan our evaluation. Thus, you will need to complete an application and send it along with copies of any other relevant materials (e.g., report cards, previous testing reports, IEPs, etc.).
You will receive:
Application for evaluation
Consent forms (to sign)
Description of evaluation
Rating scales (optional - depending on your need
Please return:
Completed application for evaluation
Signed consent form
Rating scales completed (optional - depending on your needs)
Relevant supporting documents:
Report cards
Previous evaluation reports
Relevant medical records (general medical records are not necessary)
Special education documentation (e.g., IEP, 504 Plan, etc.)
Any other documentation you feel is relevant (e.g., work samples, previous letters indicating that you received accommodations on tests, etc.).
Review of Application Materials
Once we have the application materials, they are reviewed to ensure that the evaluation is appropriate for your needs.
If the person is being evaluated via Zoom, then a short practice Zoom session may be scheduled to make sure that the person has the appropriate technology and knowledge for the evaluation.
If we determine that our evaluation may not be appropriate, then a referral to another agency or clinic can be provided.
Schedule the Evaluation
After a review of the documents, we will get in touch with you to schedule the evaluation appointments.
Evaluation
The evaluation includes multiple components that contribute to the overall cost of the assessment. These components include:
Records review
Test preparation
Testing time (virtual or in-person)
Test scoring/analysis
Client feedback session
Report writing
At approximately the midpoint of the evaluation, Dr. Van Santen will have a meeting with you to give you an update about the evaluation and let you know which aspects of the evaluation remain. If Dr. Van Santen feels that any of the evaluation components above may be more extensive or require more time than initially estimated, he will contact you to discuss the next possible steps.
Evaluation Review
After the evaluation, you and Dr. Van Santen will meet (either in person or via Zoom) to discuss the results of the evaluation. You will also receive electronic and hard copies of a comprehensive written report and test score summary (i.e., a list of tests and scores). These documents include:
a discussion of results
a description of diagnosis (if applicable)
a list of recommendations based on our findings
If requested, Dr. Van Santen can also attend any school meetings (an additional charge applies).
